We’re a team of health care providers and support staff working closely with you to provide comprehensive, team-based primary health care to residents of the Greater Hamilton Area.
Founded by the Ontario government in 2005, Family Health Teams are community- and patient-centred organisations that provide primary health care programs and services tailored to the residents of their area. The Hamilton Family Health Team is the largest Family Health Team in Ontario, serving nearly half of Hamilton’s population.
We help keep our patients healthy, treat them when they’re sick, and provide health education. You might see us in your doctor’s office, at a health care hub, in a virtual workshop, or at an event in your community. We are:
