Temporary full-time position anticipated to extend until September 20, 2024, subject to the availability of work.


Specialized Geriatric Services (SGS) provides specialized care to older adults living with frailty and their caregivers in a variety of care settings. We are also leading the Frail Seniors Strategy within Southwestern Ontario and we actively participate in
provincial initiatives aimed at improving health outcomes and the experience of older adults. In addition to providing direct care to older adults and their caregivers, St Joseph's Health Care London is also actively involved with the education of future health providers and in advancing research. Engaging patients and families as full partners in the care we provide is a strategic priority
across the organization.

We are seeking a Nurse Practitioner/Clinical Nurse Specialist to work within a collaborative practice model and a comprehensive inter-professional outpatient team in SGS. The successful candidate will provide direct, specialized, comprehensive care, and
provide clinical consultation and leadership within the Ambulatory Geriatric program. They will also participate in the education of staff and clinical partners in the clinical specialty of SGS and incorporate best practice and research findings to promote quality patient/family care. The successful candidate to this position will forecast and anticipate future trends and work collaboratively with other leaders playing an active role in influencing policy, implementing new standards and regulations, and participating in strategic planning to meet the future needs of the SGS program.


Essential Qualifications

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
  • Nurse Practitioner certificate in either Primary Health Care or Adult
  • Member of RNAO, NPAO, Canadian Nurses Protective Society (CNPS) with professional liability insurance
  • Masters of Science in Nursing (MScN) or Masters of Nursing (MN) or healthcare-related Master's degree
  • Minimum three years of recent clinical experience in delivering geriatric care in a community and/or outpatient setting
  • Superior interpersonal, coaching and communication skills to effectively work within an inter-professional ambulatory team setting
  • Demonstrated critical thinking and problem-solving skills
  • Collaborate with the inter-professional team to develop, implement and evaluate innovative patient/family-centred care delivery practices
  • Promotes evidence-based practice
  • Previous teaching experience and proven ability to provide clinical leadership
  • Experience and demonstrated skills in implementing quality improvement initiatives and leading change initiatives
  • Promote the values of shared leadership and inter-professional team collaboration
  • Demonstrated experience in project management, specifically related to nursing education and quality improvement
  • Participate and conduct research related to the Frail Seniors patient population
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

Preferred Qualifications

  • Consideration may be given to a candidate who is in the process of completing of a Primary Care or Adult Nurse Practitioner program
  • Advanced knowledge and skills with a multi-system focus in physical assessment, along with appropriate diagnostic and therapeutic considerations, consultative support and evaluation specific to the care of older adults with complex medical and functional conditions
  • Proficiency in French would be an asset

Immunization Requirements

  • Provide documentation you have received two doses of the Covid-19 vaccine and a written commitment to follow any future Public Health Recommendations and St. Joseph's Health Care London policies and Processes related to Covid management in the workplace.
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
